For confidentiality purposes, you may want to print a hard copy of this application and mail or fax it to the credit union. If you return this application electronically, we may ask you to sign a hardcopy of this application. Please complete to indicate the type of credit for which you are applying. Married applicants may apply for a separate account. If you are applying for individual credit, you must complete the Applicant section about yourself and the Co-Applicant section about your spouse if (1) you live in a community property state (AZ, CA, ID, LA, NM, NV, TX, WA, WI) or (2) your spouse will use the account. If you are applying for joint credit with another person, complete the Applicant and Co-Applicant sections.

Notice to Wisconsin Residents: No provision of any marital property agreement, unilateral statement under S.766.59, or court decree under S.766.70 will adversely affect the rights of the credit union unless the credit union has furnished a copy of the agreement, statement or decree, or has actual knowledge of its terms, before the credit is granted or the account is opened.
Notice to Married Applicants in Wisconsin. If you are applying for joint credit with someone other than your spouse, each of you must complete the Applicant section of separate applications. you promise that everything you have stated in this application
is correct to the best of your knowledge and that the above information
is a complete listing of your debts and obligations. You authorize
the credit union to obtain credit reports in connection with this
application for credit and for any update, renewal or extension
of the credit received. If you request, the credit union will
tell you the name and address of any credit bureau from which
it received a credit report on you. You understand that the credit
union will rely on both the representations you make in this application
and the contents of any credit report it obtains when deciding
whether to grant the credit requested. You agree to immediately
notify us of changes to any of the information you have provided
in this application. You understand that it is a federal crime
to willfully and deliberately provide incomplete or incorrect
information on loan applications made to Credit Unions or State
Chartered Credit Unions insured by NCUA.
